Healthy Nashville Leadership Council requests participation in visioning survey

The Healthy Nashville Leadership Council, in conjunction with numerous community partners, is in the planning phase of the Healthy Nashville Community Health Plan using the Mobilizing for Action through Planning and Partnerships (MAPP) process. MAPP is a community-wide strategic planning process for improving health. 

The vision of the group, of which VUMC is a participant, is for a healthy city that is dynamic and encompasses physical, mental, spiritual and social well-being. Focusing on the social determinants of health will help create lasting improvements for the health of everyone in Nashville. People often think of the healthcare industry when thinking of health, but building healthy communities requires strategic collaboration across all sectors. When we build a complete infrastructure of healthy communities, we can begin to close the gaps in health due to race or ethnicity, gender identity or sexual orientation, zip code or income. 

During the visioning phase, community members and local public health system partners are collaboratively determining a focus, purpose and direction for the Healthy Nashville Community Health Plan that results in a shared vision and corresponding value statements. Please take 2-3 minutes to answer just a few questions so that we can incorporate your insights.
