New “Soft Stop”: Marking the Medication List as Reviewed
As of June 9, a new “soft stop” will appear in eStar for ambulatory encounters, prompting the clinician to mark the medication list as reviewed. This change helps:
- Promote patient safety and quality care
- Bring practices in accordance with Ambulatory Prescription Renewal Protocol
- Comply with The Joint Commission’s requirement for medication reconciliation
The alert will appear if clinician has not marked the list as reviewed and will prompt the action. Marking as reviewed indicates that clinician:
- Has awareness of the patient’s medications for which the clinician is responsible
- Has deleted, added, changed, and/or communicated to the corresponding provider any medications in error
There are multiple places to mark the medication list as reviewed:
- Intake Activity
- Plan Activity
- Wrap Up Activity
- Visit Sidebar
Please note that marking the medication list as reviewed applies to face-to-face encounters with the clinician. Additionally, marking the list as reviewed does not mean that medications outside the scope of practice must be edited.
New Inpatient COVID Vaccination Process – Adult Hospital
As of June 9, there is a new process for screening, ordering and administering the COVID-19 vaccine to inpatients in the adult hospital. The process closely mirrors the existing process for flu vaccinations.
Most of the process will be handled by nursing staff, though a best practice advisory (BPA) will display for the clinician during the discharge order if the patient has not yet been offered the vaccine during the inpatient stay.
