Updates to the pharmacist-driven vancomycin monitoring process announced

The clinical chemistry lab is overhauling the analyzer used to determine vancomycin concentrations. This transition has occurred. Based on an internal analysis of patient samples, the new machine will, on average, give 2mg/L LOWER values than the old machine.

Based on these data and a review of clinical literature surrounding the safety and efficacy of different ranges of vancomycin trough values, the Department of Pharmacy will make the following changes to the monitoring process. The new goal trough ranges will be:

  • Moderately severe and severe infections: 12-17mg/L
  • Standard infections or prophylaxis: 8-12mg/L

These changes will only impact adult inpatients at VUH. There will be no changes to patient monitoring and goal ranges after discharge (e.g., skilled nursing facilities, rehab facilities or home infusion). If you have any questions, please ask your team’s pharmacist. These changes took effect in late June 2021.
