eStar updates and changes: Documenting on a deceased patient
Beginning Nov. 17, all inpatient providers will see a streamlined redesign of the Discharge as Deceased Navigator. This change will ensure complete and correct documentation for a deceased patient.
Users will be able to navigate to Cardiopulmonary Death Charting, select Signs of Death and have it populate in Create Note. When users navigate to Report of Death and then Select Document the information from the required fields will also populate in Create Note.
The Documentation section of the Navigator includes Cosign Orders for addressing outstanding orders, Synopsis, Hospital Course, and Summary of Death.
Deaths reportable under Tennessee law and Vanderbilt Policy include all those due to, related to, or admitted for reportable events as listed, regardless of interval between event and time of death. Physicians should consider reporting events if indirectly contributing to death. If there are any questions about whether case should be reported, physician completing document should call Medical Examiner.
