eStar update: Designated care contact
With VUMC’s restricted visitor policy in place, care teams need to be able to easily find a patient’s designated care contact (DCC) in eStar for communication and care coordination. A new Tip Sheet provides guidance on how to easily view the patient’s DCC in eStar.
The DCC serves as the patient’s support and primary person during an inpatient stay. The DCC receives plan of care communication and other pertinent transition information and relays this information to other family members. VUMC’s modified standard operating procedures help ensure the DCC is accurate in eStar and on the patient’s whiteboard. Inpatient users should follow the process for viewing DCC information and validation code charted by nurse.
