Adult Anticoagulation Clinic protocols updated

The Anticoagulation Clinic recently updated the anticoagulation protocols that have been approved through the required committees (Anticoagulation Oversight Committee, Pharmacy and Therapeutics and Orders Entry Committee) and made available via PolicyTech. With recommendations from the PMO, we will need to move current patients to these updated protocols and initiate the Anticoagulation Pharmacist Protocol. This pharmacist protocol will supplement our Nurse Managed protocols. Our clinic pharmacist will be able to help manage most of the warfarin-related dosing issues, and this should decrease the volume of future in-basket messages that you receive for dosing concerns.

Providers who have referred patients for Anticoagulation Management will soon be receiving an in-basket communication on each patient that we manage for them that will ask them to attest to this update. They will see in the notes sections: 1) the Attestation statement, 2) The Pharmacist Anticoagulation Protocol, and 3) the Nurse Anticoagulation Protocol specific to your patient as follows:

  • General Anticoagulation Protocol
  • Orthopedic Anticoagulation Protocol
  • Ventricular Assist Device Anticoagulation Protocol
  • Cardiac Surgery Anticoagulation Protocol
  • Left Atrial Appendage Anticoagulation Protocol

We need these attestations to continue to manage their patients that they have referred for warfarin management. We ask that you respond to our Clinical Pool basket (VUMC Adult Anticoagulation). 

An improved enrollment process in eStar will soon ask a provider to select the specific protocol to manage future patients. It will include the Pharmacist Anticoagulation Management Protocol within the enrollment. We should see this improved enrollment within the next 1-2 months.
