eStar update: Improving Last Relevant Note in Chart Review

As of July 8, the Last Relevant Note in Chart Review will be replaced with My Last Office/Hospital Note and My Last Note. This change will help provide a more holistic overview of recent documentation.

The My Last Office/Hospital Note will display the last office visit or hospital note written regarding the patient. Operative Notes are now supported.

The My Last Note will display the last note written on the patient’s chart. This information will exclude the last office visit or hospital note and can include communication with or about the patient. It allows the clinician to see the last action taken by the clinician on the patient.

These notes appear together in Snapshot of Chart Review and in some Sidebar reports. They include attested but not co-signed notes.

Refer to the Tip Sheet “Improving ‘Last Relevant Note’ in Chart Review” (available in Hubbl) for more information.
