Medication Transitions of Care Team conducting survey
As part of a hospital-wide quality improvement project to improve medication safety and reconciliation at VUH, the VUH Medication Transitions of Care Team have made improvements in pharmacy services at VUH. The team wants to learn the thoughts of employees regarding the state of medication safety and reconciliation with a survey. The short survey will take less than 10 minutes to complete and participation is confidential and voluntary.
