VUMC Limited Submission Opportunity: AstraZeneca HealthCare Foundation Connections for Cardiovascular Health; deadline Jan. 13

VUMC may submit one application to the AstraZeneca HealthCare Foundation Connections for Cardiovascular Health (CCH) program. The CCH program funds programs providing innovative, community-based approaches to improving cardiovascular health across the United States. CCH-funded programs use culturally appropriate approaches with measurable outcomes to address specific unmet needs of communities across the country, especially underserved communities.
CCH Next Generation grants of $125,000 to $150,000 for one year will be awarded to organizations that have never been funded before by the Foundation through the CCH program. CCH Next Generation has a preference for funding applications from the top 20 states by cardiovascular disease mortality, including Tennessee.
Please note that medical education and research proposals are not appropriate for this mechanism.
Priority areas:
The Foundation seeks programs that are focused on measurable results, demonstrate sustainability, and provide an innovative approach. Applications should align with one of six innovative approaches commonly used to improve cardiovascular health at the community level:
- Leveraging access for uninsured and underserved participants to improve cardiovascular knowledge and health
- Bringing programs to participants
- Educating children to serve as heart health ambassadors
- Improving cardiovascular health through food-based programs
- Using health coaches/promotores to improve cardiovascular health
- Providing culturally sensitive program interventions to maximize participant outcomes
Applicants are encouraged to consult the AstraZeneca HealthCare Foundation’s Effective Program Practices Guide to help them design their own community-based cardiovascular programs.
Eligibility:
Applicants must be engaged in charitable work (at the community level or otherwise) that addresses the Foundation’s CCH mission and meets five key criteria:
- Addresses cardiovascular health issues within the United States and its territories
- Recognizes and works to address an unmet need related to cardiovascular health in the community
- Responds to the urgency around addressing cardiovascular health issues, including cardiovascular disease or conditions contributing to cardiovascular disease
- Improves the quality of participants’ and non-professional caregivers’ lives in connection with the services provided and work done
- Demonstrates a clear plan for long-term program sustainability, to further improve cardiovascular health after Foundation grants are expended.
All programs should be designed to reach at least 400 unduplicated participants by year-end, and to track at least 200 unduplicated participants by year-end.

Internal Application Process:
Anyone interested in being considered to submit VUMC’s proposal must submit the following (in a single PDF) to LSO@vanderbilt.edu by 5 p.m. on Jan. 13, 2020:
- Brief (2-page maximum) research plan including summary budget;
- Statement of support from department chair/center director;
- NIH Biosketch or 5-page CV.
