eStar updates and changes: ordering infusions for titration and documenting COVID vaccines given outside VUMC system

Ordering Infusions for Titration

Beginning April 28, there will be a Format change for ordering infusions for titration. The newly assigned order questions are present in the Order Composer to assist with the ease of placing infusion orders. These changes are necessary to create more efficient workflows for ordering and administering titrations, and to drive compliance with Joint Commission’s requirement for titrated infusions.

Documenting COVID-19 Vaccinations Received Outside VUMC

Many patients are reporting that they have received COVID-19 vaccinations outside VUMC, which should be documented in the patients’ immunization records. The guidance below represents best practice to document this information.

The most efficient, accurate approach to document this to first check if there is COVID-19 vaccination information from latest query to TennIIS (Tennessee Immunization Registry) database and to reconcile data. From the Immunizations activity, users should navigate to Administration History  and then to Immunizations from Immunization Registries.

If the COVID vaccination information does not appear, the user should double-check the date/ timestamp of most recent query. If that date/time precedes the patient’s reported COVID vaccine, the user can click Imm Registry link to trigger a real-time query to TennIIS, and results can be received within seconds. Outside vaccinations can then be easily reconciled via Go Reconcile button on banner

If no COVID vaccine information appears even after a new query, COVID vaccines can be manually added to the patient’s chart via Single Historical Immunization on Historical Admins menu (Immunizations toolbar).

When manually entering COVID vaccination, do not use “unspecified”. If patients are unsure which type of vaccine they received, this can be determined by assessing number of doses required and number of weeks scheduled between doses. Clinicians must be vigilant about accurately entering type of vaccine and date of administration, and may enter additional information if available.
