Adult Medicine VTE CDS Revisions
Beginning November 10, inpatient VUH providers will be able to easily access the DVT/VTE Prophylaxis order entry for the Adult Medicine patient population.
Improved VTE clinical decision support has been developed for Adult Medicine patients. The Adult Medicine VTE Prophylaxis best practice advisory has been suppressed for more patient populations and includes a simplified menu of Acknowledgement Reasons with different time intervals for snoozing the best practice advisory. A new permanent contraindication order enables appropriate documentation for patients who should not be prompted to receive anticoagulation during the encounter.
IDDSI terms and IDDSI diet orders for dysphagia
Beginning November 9, the new International Dysphagia Diet Standardisation Initiative (IDDSI) will replace the National Dysphagia Diet (NDD). IDDSI terms and IDDSI diet orders will be used for dysphagia in eStar.
SLPs recommend two dysphagia orders of food consistency and liquid consistency. Mechanically altered diet orders will still be available for other patients who need texture modifications, but do not have “dysphagia” diagnosis. New Epic IDDSI terms will be available. The consistencies are either similar or the same. The IDDSI food and liquid order will print on patients’ tray tickets.
Some liquids are purchased and served on tray as “pre-thickened.” Only these pre-thickened liquids will be sent on patient trays. No “thin” or unthickened liquids will be sent. All other liquids and water need to be thickened by RNs on the floor/unit.
Mixing instructions for Thicken Up are part of Epic liquid consistency orders. Instructions to mix Thicken Up packets are the same. Terms used for thickened liquids will now be IDDSI terms. IDDSI terms for thickened liquid. Mildly Thick used to be called Nectar Thick. Moderately Thick used to be called Honey Thick. Extremely Thick used to be called Pudding Thick.
