eStar update: New criteria for renewals of long-term medications
Beginning June 10, there are new requirements for the renewal of long-term medications via the Ambulatory Renewal order mode. These new requirements will help enhance patient safety and ensure compliance with updated policy. The change will impact all ambulatory nurses, clinicians and pharmacy staff.
The new criteria to be completed are:
- Medications must be marked as “taking long-term” (most medications prescribed for more than one year are marked as such by default)
- Medication list must be “marked as reviewed” by a clinician within last year (may be any clinician)
If the medication renewal does not meet criteria, the nurse or pharmacy staff must route the order to the prescribing clinician for review. The prescriber will receive an In Basket message prompting review of the reasons for the failure of the medication’s renewal. The prescriber can then correct these issues.

- These should be used together exclusively
- Medications on this protocol are same as previous standing order
- Some new medication groups have been added (see list-must be on VUMC network to view)
- Medication additions can be requested through local medical director (who will place the request through PT&D)
