Feedback requested from adults with Type 1 diabetes who transitioned from pediatric to adult care in past 10 years

Individuals with Type 1 diabetes between the ages of 18-28 are invited to give feedback on the challenges of transitioning from pediatric to adult care providers. Researchers will discuss your thoughts on how communication, supports and other aspects of care can be improved to make transitioning providers easier for all patients. What: A two- hour, small group meeting with a meal provided. All participants will receive a $50 gift card for their participation.

Who: Adults who have transitioned from a pediatric to an adult healthcare provider in the last 1-10 years.

When: TBD during April based on participants availability.
