eStar updates and changes announced
New Intake Questions
Beginning April 24, new questions will be added to the intake questionnaire that will address a patient’s nutrition and physical activity. The questions have been added to further operationalize a patient-centered health model. They will also provide behavioral data for future clinical and community health initiatives. The changes will impact all individuals, including clinicians and nurses, who complete or review the intake screening questionnaire.

New Step for Medication Reconciliation at Admission
Beginning April 24, eStar will now prompt inpatient clinicians to “Review Home Medications” (Tab 1) as part of the admission medication reconciliation workflow. Previously, eStar skipped to “Review Current Orders” (Tab 2). This change will help ensure the review and accuracy of prior-to-admission medications before the clinician makes decisions (e.g., order, don’t order, replace, or discontinue each medication).
