Update 1: New FYI Flags for Shared Plans of Care
To enhance care coordination across Vanderbilt, new shared plans of care will be available in FYI flags beginning Sept. 18, 2019.
If a patient has a shared plan of care, a Best Practice Advisory (BPA) will fire when opening the patient chart. This functionality expands on the “Familiar Faces” FYI Flag and FYI Flags in Shared Plan of Care include:
- Familiar Faces
- Care Transitions Plan
- Post-Acute Care Transfer Ticket
Shared Plans of Care are personalized care plans created by clinical teams with the most comprehensive knowledge of the patient. These plans will be shared transparently within eStar for all team members to access. They are visible across encounters and through Vanderbilt Health Connect. Shared Plans of Care will be created and updated by specific groups or areas working with patients who have complex or ongoing care coordination needs.

Update 2: New Process Instructions for Cryorecipitate
To guide appropriate dosing for patients, new process instructions for cryorecipitate ordering will be live on Sept. 18, 2019.
